Allied Health Solutions focuses on providing allied health services to people who are living with or are at risk of developing a chronic disease.
The highly-qualified Care Coordinators will advocate for your health needs and arrange a team-based approach to your care.
The Care Coordinators will support you to access a diverse team of Allied Health Professionals, such as Physiotherapists, Dietitians, Exercise Physiologists and Podiatrists, at no cost.
Together with your GP, we will find ways to help you manage your chronic medical conditions.

am i eligible?
To be eligible for this program, you must:
- Live in the Southern Yorke Peninsula or Lower North regions, and;
- Be living with, or at risk of developing an ongoing chronic medical conditions that requires additional services to prevent further complications or illness.
What are the costs to participate in the program?
All assessments, pre and post program, and the individual sessions are completely free to eligible participants. You must fit one of the criteria/conditions listed above, and be referred by your GP.
How can I participate in the program?
To access this service, simply contact the friendly team at Sonder (08) 8209 0700 or email [email protected]
Sonder will put you in touch with a local GP to access a referral into the program. If the GP finds the program to be suitable for you, the GP will then complete the Referral Form and send it back to Sonder.
The Sonder Care Coordinator will work with your GP to coordinate initial appointments with the specialist nurse, or Allied Health Professionals identified on the referral.

We will be providing this funded program in Balaklava and Riverton.
