Please arrive 5 minutes early to your scheduled appointment time to allow you to complete a new patient form. This allows us to gather background details and information from you in preparation for your appointment.
What to bring to your appointment:
What to wear?
Please wear comfortable clothing that you can move freely in. Please also wear comfortable, closed toe shoes that you can move about well in. You may be asked to undertake a movement or walking assessment.
If you will be undertaking a supervised training session in the clinic, please also bring a sweat/hand towel and a drink bottle.
Please allow 40-45 minutes for your first appointment to allow enough time to undertake all assessments required so that we can best help you.
A minimum of 24 hours notice is required if you are unable to keep your appointment. Failure to attend or advise that you are unable to keep this appointment my incur a non-attendance fee.
Fees
You are required to pay your account in full at the time of consultation. Payments can be made by cash, credit card or debit card.
Private Clients
Rebates are available on our services through your private health insurance (depending on your level of cover) and we can process your rebate on the spot if the fund is registered for HICAPS. If your health fund is not registered for HICAPS, we will supply you with a receipt to claim your rebate back. Please be aware that amounts vary between insurance companies and we recommend that you contact your insurer for more information regarding your individual cover.
Clients referred under the Medicare Chronic Disease Management Scheme or Group Diabetes Program
If you have been referred by your doctor under the Chronic Disease Management Scheme/Diabetes Group Exercise program you are able to receive a rebate from Medicare for your appointment and this will be submitted direct to Medicare for you at the time of your consultation. Rebates are generally received back into your bank account within 24 hours. Depending on the type of appointment you may be left with a gap of between $7.00 and $19.00.
Department of Veteran's Affairs/Return to Work SA
Department of Veteran's Affairs and Return to Work SA deal directly with us, and if your service is covered by these providers there will be no fee for you to pay.

Our Clare Clinic is located at 6/25 Old North Road, Clare. We are located behind Target, between service SA and Goodyear Auto Care and face the car park. Parking is available straight out front and there is a disabled parking space available.

- a copy of your referral if you have been referred by your GP, or other allied health provider (If we rang you to make an appointment, we already have a copy of this therefore you do not need to bring one)
- reports from any recent scans or xrays which are in relation to the conditions which you are seeking support from your exercise physiologist.
- your medicare card (if referred by your GP under a care plan)
- your concession card (if you have one)
- If you are attending for cardiac rehabilitation and have been prescribed an angina medication/spray. Please bring this with you to your appointment.
