Telehealth
Telehealth allows our clients to access our service and support, without having to attend our clinic. This can be utilised for our Exercise Physiology or Diabetes Education Services.
How Does Telehealth work?
1. Make an Appointment
You can make an appointment by calling 8873 0628. You can decide to do this yourself privately or by obtaining a Chronic Disease Management (CDM) plan from your GP or DVA referral, or ask your specialist to refer you.
2. Initial Consultation
Before your appointment, you’ll receive some automated emails and texts giving you information, support and instruction about your telehealth appointment. At your first consultation, we’ll talk through your goals for your exercise physiology or diabetes care and recommend the best next step. We can even show you video of exercises as we chat so that you are familiar with your exercise program before you start (if it is an exercise physiology appointment).
3. Data Uploads
We’re happy with a high-tech or low-tech approach to reviewing your progress. We often prescribe exercise programs using an online platform that we can both access, and if for Diabetes Education you can also upload your blood glucose data to a secure patient portal that we can access, otherwise we are also fine with paper records for both exercise and blood glucose monitoring - you can simply scan, or send us a photo prior to your appointment.
4. Manage Your Health Your Way
We’ll provide you with tools and strategies and a detailed management plan so that you know exactly where you are heading following our appointment. This will fit with your lifestyle, health, ability and goals to improve your health management. We tailor our evidence-based advice to suit you and your life.
5. Payment
Payment is due in full at the end of your consultation. We’ll ask you for your card details and process your payment using our EFTPOS machine.
If you have a Chronic Disease Management Plan, please present this to us before your first appointment as we may be able to claim these item numbers on your behalf if the care plan visits are assigned to our providers.
Alternatively you can present your paid invoice to Medicare or to your private health fund as you may be able to claim a rebate for our services, which is usually paid into your nominated bank account within 24 hours.
6. Communication
We are here for you. You will be provided with contact numbers and emails to contact us at any time. We will also correspond with other members of your healthcare team (with your permission) such as your GP and specialist after our appointment so that we are all on the same page with your care.
What Does a Telehealth Appointment Cost?
A private fee is charged for telehealth appointments, except for certain items on a Chronic Disease Management Plan from your GP where we charge a gap fee, or there is no gap for DVA clients.
Payment is due in full at the end of your consultation. If the care plan referral is sent to reception in advance, we may be able to claim the rebate on your behalf, Medicare will then deposit the rebate directly into your account in 24-48 hours. If you have been unable to provide your care plan referral from your GP, you can then present your paid invoice to Medicare or your health fund as you may be eligible for a rebate, which is usually paid into your bank account within 24-48 hours.
What technology do I need to be able to access telehealth?
You simply need a smart phone/tablet/ipad with the ability to download free apps, or a computer with sound and a camera. We will provide you with all the information about the free program for you to use after you have booked your appointment.
